We work with most major PPO plans and Medicare. Coverage varies by product line, so please call to confirm your specific plan before your visit.
Referrals
Some HMO and Medicare Advantage plans require a referral from your primary care physician. Your insurer's member services line can tell you in seconds.
What to bring
A photo ID and your insurance card. If you have prior records, imaging, or lab results, bring those too.
